Opening an account with Frontier Imaging, Inc. is simple. Complete the New Customer Account Request Form and then select the type of account term you wish to use, then complete and submit the application forms under the following payment options:

    a.     Account/Credit Application Form
    b.     Bank Authorization Form
    c. Provide a copy of your resale certificate

  2. CREDIT CARD PAYMENT TERM
    a. Account/Credit Application Form
    b. Credit Card Authorization Form
    c. Provide a copy of your resale certificate

  3. PREPAYMENT BY CHECK OR WIRE TRANSFERS (T/T)
    a. Account/Credit Application Form
    b. Provide a copy of your resale certificate
